ESC @ GW-ICC Joint Forum – Great Wall International Congress of Cardiology (GW-ICC) 2017- Interventional Cardiology and Acute Coronary Syndromes

It has become a tradition for world renowned cardiology associations to send delegations to GW-ICC congresses.

Professor Jeroen J. Bax, President, European Society of Cardiology (ESC), Professor John Warner, President, American Heart Association (AHA), and Professor Mary N. Walsh, President, American College of Cardiology (ACC), led delegations attending the 28th GW-ICC, to present latest international guidance, cutting-edge diagnosis and information on treatment with emerging technologies.

Cooperation with these international societies is not limited to their attendance at GW-ICC; GW-ICC also holds reciprocal academic sessions at the annual meetings of these eminent societies, for example GW-ICC @ ACC, GW-ICC @ AHA, and GW @ ESC, to introduce important Chinese academic research.

At the 28th GW-ICC in 2017, the ESC @ GW-ICC Joint Forum, which was held over four sessions, focused on the highlights of 2017 ESC Congress, to introduce the latest guidance and discuss the latest developments of ESC images, heart disease, heart failure, STEMI and valve disease.

A video of one of these 2017 sessions is now available to view on the GW-ICC YouTube channel:

Interventional Cardiology and Acute Coronary Syndromes

Presenter: Marco Roffi, Geneva University Hospitals, Geneva, Switzerland.

In this presentation Professor Roffi discusses reviews Interventional Cardiology and Acute Coronary Syndromes topics from ESC 2017 with the following take home messages:

  • Decision about the optimal revascularization strategy of complex multivessel CAD should take into account age and clinical profile; anatomic complexity and probability for achieving complete functional revascularization, and access to contemporary drug eluting stents.
  • With contemporary PCI, de-escalation of long term antithrombotic therapy either chronic DAPT in ACS or dual therapy among selected patients with atrial fibrillation appears to be safe and associated with net clinical benefit.
  • The ESC 0/1h algorithm for rapid rule out/in of acute MI is safe and effective and should be extensively implemented.
  • Patients with ACS are changing over time: and older age and more comorbidities may account for complications and a worse outcome.
  • Younger women with STEMI have a higher mortality as compared with men, even when they receive the appropriate treatment in a timely fashion.

KSCMS @ GW-ICC Joint Forum – Great Wall International Congress of Cardiology (GW-ICC) 2017 – Practical management of Hypertension in Korean after SPRINT trial

It has become a tradition for world renowned cardiology associations to send delegations to GW-ICC congresses. Cooperation with these international societies is not limited to their attendance at GW-ICC; GW-ICC also holds reciprocal academic sessions at the annual meetings of these eminent societies to introduce important Chinese academic research.

At the 28th GW-ICC in 2017, the Korean Society of Cardio metabolic Syndrome (KSCMS) held a Joint Forum. A video of one of these 2017 sessions is now available to view on the GW-ICC YouTube channel:

Practical management of Hypertension in Korean after SPRINT trial

Presenter: Sang-Hyun Ihm, MD, PhD, Department of Internal Medicine, Catholic University of Korea, Seoul, South Korea.

Dr Sang-Hyun Ihm discusses:

  • Target BP of recent guidelines.
  • Observational studies – lower is better.
  • Updated systemic review and meta analysis.
  • Systolic blood pressure intervention trial (SPRINT).
  • Several issues of SPRINT.

Concluding that:

  • Since AOBP is not generally available in clinical practice, the definition of hypertension should remain BPs >140/90mm Hg.
  • AOBP will be the preferred method of performing in-office BP measurement
  • The target BP should remain to lower BP in most hypertensive patients to <140/90 mmHG.
  • The higher cardiovascular risk: SPRINT eligible patients
    • Target SBP should be <120mmHG using AOBP
  • Others including DM, CKD with overt proteinuria
    • Target BPs should not be changed
  • The balance between benefits and risks should be taken into account for the practice of personalized medicine.
    • If tolerated well, one could attempt to go lower incrementally with careful monitoring.

Presenter: Steven R. Houser PhD, FAHA, Lewis Katz School of Medicine, Temple University, Philadelphia, PA, USA.

Professor Houser discusses the following:

  • How can the heart be repaired following MI to prevent HF from developing?
  • A treatment that can cause replacement of injured/dead tissue with normal heart is the ‘holy grail’ of the field.
  • In the presenter’s view, there is no effective regenerative therapy at this time.
  • The presenters research has identified a cell, derived from bone stroma that has characteristics that should aid the repair of the heart after MI.
  • The cell is undergoing further testing at present.

ESC Congress 2017 Highlights

This presentation covering highlights from preventive cardiology sessions at ESC 2017 discusses how:

  • Links between saturated fats, carbohydrates and outcomes are still puzzling and more clarification is needed, including in guideline recommendations;
  • Triple vascular screening on the population level seems effective in reducing all-cause mortality. Cost effectiveness indicates it is worthwhile implementing;
  • CETP inhibition is a new LDL lowering medication with effect on CVD events;
  • PCSK9 inhibition is safe, reduces plaque volume but does not change plaque composition.

American Heart Association (AHA)

Session title: Hypertension 2018: Understanding Genetics, Prevention Strategies and Management

Over a billion people around the world have hypertension, which is said to be the most important risk factor for premature cardiovascular disease. The root causes of hypertension remain unknown. In this session Ivor Benjamin, Professor of Medicine and Director of the Cardiovascular Center at Froedtert & the Medical College of Wisconsin, WI, USA, will discuss studies that are helping us understand genetic origins of high blood pressure and Sidney C. Smith, Jr., Professor of Medicine and Director of the Center for Cardiovascular Science and Medicine at the University of North Carolina, Chapel Hill, NC, USA, will discuss treatment strategies for patients with confirmed hypertension according to the new AHA/ACC hypertension guidelines.

Korean Society of Cardiometabolic Syndrome (KSCMS)

Session title: State-of-Art Cardiometabolic Syndrome

Adequate prevention and treatment are important for prevention of cardiovascular events, development of type 2 diabetes, and dementia. Recent national studies from Korea and USA reported that the prevalence of cardiometabolic syndromes is stable, but not decreasing. State-of-Art pathogenesis, prevention, and treatment will be discussed during this session.

Japanese Circulation Society (JCS)

Session title: Heart Failure Topics – from molecular mechanisms to cutting-edge treatment

Numbers of heart failure patients is rapidly increasing globally. This session will present topics related to heart failure including molecular mechanisms and cutting-edge treatments.

Turkish Atherosclerosis Society (TAS)

Session title: Clinical Links of Pathogenetic Mechanisms in Atherosclerosis

Atherosclerosis is usually silent until the development of a cardiovascular event. Cardiovascular events generally occur before atherosclerosis is detected. The detection of atherosclerotic processes and its related clinical importance is therefore of great significance for prevention. The aim of this session is to evaluate some underlying causes of this condition and their impact on the development of atherosclerosis.

European Heart Journal (EHJ)

Session title: The Year in Cardiology – the EHJ Perspective

Transcatheter Cardiovascular Therapeutics (TCT)

Session title: 30th TCT Anniversary Perspectives of Interventional Cardiology
