WHF @ GW-ICC Joint Forum – Great Wall International Congress of Cardiology (GW-ICC) 2017 – Diastolic Function: the Importance of Untwisting and Suction in Diastolic Filling

It has become a tradition for world renowned cardiology associations to send delegations to GW-ICC congresses.

Cooperation with these international societies is not limited to their attendance at GW-ICC; GW-ICC also holds reciprocal academic sessions at the annual meetings of these eminent societies, for example GW-ICC @ ACC, GW-ICC @ AHA, and GW @ ESC, to introduce important Chinese academic research.

Diastolic Function: the Importance of Untwisting and Suction in Diastolic Filling

Presenter: John E. Sanderson, MA MD FRCP

Professor Sanderson discusses how:

  • Untwisting with recoil causes suction which is responsible for pulling the blood into the ventricle during early diastole.
  • The extent and speed of the recoil and hence suction depend on the force of the previous systolic contraction and the amount of stored energy, rate of myocyte relaxation, state of titins within the myocyte and the quality of the extracellular matrix.
  • Reduced suction, and hence reduced early diastolic filling is found in nearly all forms of heart failure and is associated with normal aging. Exercise may slow the rate of reduction.
  • Improving this fundamental property of the heart as a suction pump will require sophisticated therapy.
  • Systole and diastole are intimately connected and there is in essence one cardiac cycle. Isolated diastolic dysfunction is extremely rare.

ESC @ GW-ICC Joint Forum – Great Wall International Congress of Cardiology (GW-ICC) 2017 – Imaging

It has become a tradition for world renowned cardiology associations to send delegations to GW-ICC congresses.

Professor Jeroen J. Bax, President, European Society of Cardiology (ESC), Professor John Warner, President, American Heart Association (AHA), and Professor Mary N. Walsh, President, American College of Cardiology (ACC), led delegations attending the 28th GW-ICC, to present latest international guidance, cutting-edge diagnosis and information on treatment with emerging technologies.

Cooperation with these international societies is not limited to their attendance at GW-ICC; GW-ICC also holds reciprocal academic sessions at the annual meetings of these eminent societies, for example GW-ICC @ ACC, GW-ICC @ AHA, and GW @ ESC, to introduce important Chinese academic research.

At the 28th GW-ICC in 2017, the ESC @ GW-ICC Joint Forum, which was held over four sessions, focused on the highlights of 2017 ESC Congress, to introduce the latest guidance and discuss the latest developments of ESC images, heart disease, heart failure, STEMI and valve disease.

After qualifying in medicine at the University of Barcelona, Dr Fuster continued his studies in the United States. Wiener Cardiovascular Institute and the Marie-Josée and Henry R. Kravis Center for Cardiovascular Health and is CEO of the National Center for Cardiovascular Research (CNIC). He is also the Richard Gorlin, MD/Heart Research Foundation Professor, Icahn School of Medicine at Mount Sinai. Dr Fuster has made significant contributions throughout his professional career to the advancement of cardiology research, the prevention and treatment of cardiovascular disease, particularly coronary artery disease, atherosclerosis and thrombosis.

Dr Joseph Hill is Chief of UT Southwestern Medical Center’s Division of Cardiology and Director of the Harry S. Moss Heart Center, Dallas, TX, USA. Dr Hill’s research focuses on molecular mechanisms of structural, functional, and electrophysiological remodeling in cardiac hypertrophy and heart failure. He earned his medical and doctoral (pharmacology) degrees at Duke University, completed fellowships with the Institut Pasteur in Paris and the Brigham and Women’s Hospital, Harvard Medical School.

Prior to his current position he was Professor of Pharmacotherapy at the University of Basel, then Professor and vice-chairman of cardiology at the Inselspital of the University of Berne, Switzerland.

Non-Statin Therapy- What is the Evidence and When Do We Use Them?

Presenter: Christie M. Ballantyne, Center for Cardiovascular Disease Prevention, Houston Methodist DeBakey Heart & Vascular Center, Baylor College of Medicine, Houston, TX, USA.

Dr Ballantyne discusses how:

  • Evidence from clinical trials have conclusively shown the benefit of aggressive lowering of LDL-C in high risk individuals and all guidelines recommend initial therapy with statins.
  • Asian patients must be included in clinical trials for lipid therapy as both risks and benefits have been shown to be different than western populations (i.e. statins and niacin)
  • Nonstatin therapies (ezetimibe, PSCK9 inhibitors and anacetrabib) have now been shown to reduce CVD events when added to statin therapies consistent with the amount of LDL-C reduction and with safety data in Asian patients.

Roadmaps: What, When, Where, Why, and How?

Presenter: Daniel José Piñeiro, Professor of Medicine, Universidad de Buenos Aires, Buenos Aires, Argentina.

Professor Piñeiro discusses how the World Heart Federation has undertaken an initiative to develop a series of Roadmaps, such as The World Heart Federation Roadmap for NonValvular Atrial Fibrillation, to promote development of national policies and health systems approaches, and to identify potential roadblocks on the road to effective prevention, detection, and management of cardiovascular disease in low-and middle-income countries (LMICs) and develop strategies for overcoming these.

Statins 2017 Pitavastatin

Presenter: Meral Kayıkcıoğlu, Ege University Medical School, Izmir, Turkey.

In this presentation Professor Kayıkcıoğlu reviews Pitavastatin:

  • Pitavastatin has a novel structure compared to other statins
  • The cyclopropyl side chain:
    • Allows tight binding to HMG-CoA reductase
    • Diverts metabolism away from CYP3A4 through which many other drugs are metabolised
    • May reduce the extent of pitavastatin metabolism
  • Overall safety profile is typical of the class
  • Post marketing data from Japan shows a comparable muscle ADR to low dose simvastatin
    • Has pleiotropic effects
    • Effect on HDL-C is remarkable
    • With high safety
