Cerebral Protection in TAVR

Presenter: Professor Gerhard C. Schuler, Herzzentrum, Universitätsklinik für Kardiologie, Leipzig, Germany.

Professor Schuler discusses:

  • How stroke remains a major TAVR complication, which increases mortality by threefold.
  • A recent SAVR study has demonstrated a 17% stoke rate with neurologist performing NIHSS assessment.

The Watchman – Indications and Clinical Update

Presenter: Kwan S. Lee, MD, FACC, FSCAI, Professor of Cardiology, University of Arizona, AZ, USA.

In this presentation Professor Lee discusses how:

  • Left atrial occlusion with a Watchman device implanted with the appropriate training is safe;
  • The best data is for patients who can take short term warfarin;
  • There is reasonable registry data for patients who cannot take long term anticoagulation and a randomized trial is pending;
  • Cost is significant and cost efficacy will ultimately decide widespread use.

How to Use Statins to Maximize Cardiometabolic Benefits

Presenter: Kwang Kon Koh, MD, PhD, FACC, Gachon University of Korea, Incheon, South Korea.

Dr Kon Koh discusses statin guidelines to maximise cardiometabolic benefits:

Primary prevention

  • Without risk factors for diabetes: low (for Asian) or optimal (for Caucasian) dose statins alone.
  • Without risk factors for diabetes: low or optimal dose statins +/- ezetimbe combined with RAAS blockades or PPARa agonists to reduce diabetogenic effects of statins.

Secondary prevention

  • In acute coronary syndrome state: potent high dose statins +/- ezetimbe/PCSK9 inhibitors/inclisiran because cardiovascular benefits of statins exceed diabetogenic or other risks.
  • In stable coronary artery diseases: optimal dose statins +/- ezetimbe combined with RAAS blockades or PPARa
