Timing of Complete Revascularization in STEMI: Insights From the OPTION–STEMI Trial

This editorial examines the findings of the OPTION–STEMI trial and their implications for the management of patients with ST-segment elevation myocardial infarction (STEMI) and multivessel coronary artery disease.

While current guidelines recommend complete revascularization, the trial investigated whether non-culprit lesions should be treated immediately or in a staged procedure during the same hospital admission. The study found that immediate complete revascularization did not demonstrate non-inferiority compared with staged treatment.

Notably, patients with acute heart failure showed worse outcomes with immediate intervention, suggesting that a staged approach during the index hospitalization may be safer for higher-risk patients. (more…)
